TRANSCRIPT ORDERING FOR FORMER BOONE GRADUATES

• We must have a written request. Your signature authorizing the release of the transcript is mandatory.  You have three options: (1) Write a letter giving Boone the authorization, (2) Mail the form on the Boone Website, or (3) Use the form in the College & Career Office at Boone. 
• Provide the following information:  Your official school name, your maiden name if applicable, your present address and phone number, your birthdate, year of graduation, student number if you remember it correctly, the institution(s) to which you want the transcript sent, and the address of the institution(s).
• Remember – there is a $2.00 fee for each transcript we send unless the transcript can be sent electronically.  We can only send electronic transcripts to the four-year Florida colleges/universities, EXCEPT Florida Gulf Coast and North Florida. Electronically sent transcripts are free.  We are unable to electronically send to Valencia Community College campuses. If the transcript is being sent to any private college/university or out-of-state school the fee is also $2.00.  The fee can be paid by cash or money order ONLY.
• Remember- when your request is received, we place the order, and when we receive the transcript it is mailed.  There is a 3-5 day period between ordering and mailing the transcript.
• If you have an immediate need for a transcript call the Orange County Public School Records Office at 407-317-3965 or you may visit their office and get one while you wait. Their office is located at 6501 Magic Way in Orlando.  It is at the intersection of John Young Parkway and Oak Ridge Road.  Going west on Oak Ridge road, you make a left onto John Young Parkway and Magic Way is the first driveway on the right.  Go to the south end of the 500 building for entrance to the Records Office.
• We can only order transcripts for students graduating from Boone after 1990.  All others must contact the OCPS Records Office.
• Remember – the envelope of an official transcript cannot be opened.
